Do i need glasses? i cant tell?

ok so i have never really had trouble with my eyes until now i am 13 well my eyes itch and hurt all the time really and anything faraway or close up is really hard to focus on especially when it is up close the object i am looking at when it is up close it doubles and gets blurry also if any writing is in small print and is close together like words in a book its hard for me to focus on stuff this isn't really a major thing its just i am wondering do i need to go see an eye doctor i have went before and i am able to read those charts but they are easy to read if you give me something like a book to read i will have trouble or an object close up or faraway should i go see an i doctor ?
