Do I need to submit multiple Instructor Evaluation forms?

I am applying to transfer to another college via College App and my adviser said that i need to print out multiple Instructor Evaluation forms and give them to my different professors. Is that actually necessary? It doesn't really make sense to me because if I was transferring after 2 years I would need to possibly track down around 20 professors and get copies for each college i applied to.
